When needed or 21: Most women do not need a vaginal exam until 21 or as soon as sexually active to screen for stds & abnormal cytology (pap smear). Other reasons for an earlier exam would be for symptoms of infection, abnormal bleeding, or pain. Current standards are increasing the intervals between exams or delaying initial exams but it's probably best to discuss this with your gyn to see what's best for you.
