See your doctor: Anal fissures can be healed over time with sitz baths using epson salts, sometimes Nitroglycerin cream, and attention to bowel habits (keeping stools soft). Others may need surgical intervention such as excision or internal sphincterotomy. To make sure you do not have an infection there, you should have a physical exam with your doctor.

Perirectal abscess: Anal fissures are painful, but rarely cause systemic signs of infection. I would be concerned that you have developed a deeper infection, possible perirectal, or elsewhere, to explain fevers and chills. You should see a physician for an examination as soon as possible.
