Lack of harmony: Affect or mood exhibited by the patient , usually goes hand in hand with the thought content expressed by speech. So incongruity means there is absence of that harmony or congruity, so the patient is describing a genuinely painful and sad, or fearful experience and feeling, while exhibiting feelings of joy and exuberance ..And visa versa..It is most suggestive of schizophrenia.
