Yes: Endometriosis is diagnosed by surgery, however, if implants are not large it can be missed. That being said, there are other reasons for painful heavy periods. Often uterine fibroids or pelvic vein congestion can cause heavy and painful cycles. There are several treatments, both hormone free and horomonal. You should discuss treatment options with your doctor.

Painful Menses: If your painful menses are not relieved with 600mg Motrin then it is not dysmenorrhea. In other words painful periods due to prostaglandins. Even though the surgery did not see classic endometriosis, you may still have it as well as adenomyosis or endometriosis in the uterine muscle. Discuss oral contraceptives, lupron, (leuprolide) zoladex.
