Lots of causes: The good news is a 21 yr old's risk of heart attack is very very small. The list of possible causes is big. Some examples are muscle strain, injured rib or cartilage, pleurisy, gall bladder, liver, kidney stone, pulmonary embolus, and others. You should get it checked out.
