Yes and yes: He may be hard of hearing or he may be tuning you guys out. It matters whether he is able to hear other sounds without difficulty. Children around this age can also be so engrossed by the task at hand that other stimuli are ignored. Schools also have a yearly hearing screen that looks for substantial hearing loss. If you are very worried, an otolaryngology or audiology consultation is indicated.
