Swelling: "puffy eyelids" can refer to swelling of the tissues around the eyes from numerous causes, including allergy, edema (fluid retention), sinus disease, and even infection. Some people will have prolapse of orbital fat herniating into the eyelids causing a continually puffy appearance. Thyroid related orbital disease can also cause lid swelling.
