Joint or nerve pain: It sounds like you have osteoarthritis of hip and lower back. The 2 most distal rows of (furthest from the arms) finger joints and the base of the thumb are most often effected by osteoarthritis. However, a "flash" of pain may be from median (carpal tunnel syndrome) or ulnar nerve compression. You could see a rheumatologist or hand orthopedist.
