Ear infections: The criteria that doctors typically use for referral is 4 ear infections in 6m. So you say multiple ear infections, has it been 2 ear infection or 3 ear infections? It also depends on your insurance, do you need a referral or can you self refer?

Insurance?: It depends on your insurance. If you have a PPO, you can just make your own appointment with an ENT. If you have an HMO, you need a referral, but you can make an appointment with an ENT and pay out of pocket. You could also change pediatricians or just continue to insist that you would like to see an ENT. Also, if you have concerns about hearing loss, you can ask for a hearing test.
