Massive heart attack: There is no fixed number. Presently most heart attacks can be aborted by opening the culprit artery with angioplasty or heart surgery. Medical help must be sought in a timely fashion.

Not predictable: Massive is unclear some bad ekgs and high enzymes may not be accurate as to remaining muscle. When damage causes low function, low ejection fraction, and congestive heart failure then one may need left ventricular assist lvad or transplant if a candidate.
