See a Neurologist: Vision loss and speech difficulties associated with migraine elevate the classification of your disorder to complicated migraine. Nausea/vomiting, photophobia, phonophobia are frequent symptoms accompaning migraine. Typically complicated migraine is not responsive to otc medications, but there are medications when given at the onset of the headache that can abort it. You should see a neurologist.

Yes: Normally migraines do not cause any permanent vision loss. Trouble with speech and vomiting could be a sign of elevated intracranial pressure, or a stroke, so seek medical attention at the er right away.
