Pain left upper abd: Splenic rupture can occur with injury to the left upper quadrant of the abdomen (below left side of rib cage), which is where the spleen is located. With severe internal bleeding, there can also be lightheadedness esp when standing, fainting, trouble breathing. The only way to tell for sure is to be scanned. Any injury bad enough to warrant fear of splenic injury warrants emergency evaluation.
