Through blood work: Which include hepatitis b surface antigen, hepatitis b surface antibody, hepatis b core antibody, hepatitis b e antigen, hepatitis b viral load etc. Depending on the combination of these etc, you can tell if its an acute infection or whether the person is already immune.
