By your cardiologist: Heart transplantation is a last resort measure -- so you have to have had an extensive evaluation by your cardiologist and he will determine whether you are qualified to be on the list for a heart transplantation.

Why?: You don't just get a transplant. If you have severe heart failure you need treatment for that with medicines. You need to be under the care of specialized cardiologist for a long time. Transplant is the last resort if you reach a stage you can't survive in spite of all the efforts to control your hf at rest.
