If : If the proper treatment with medicines cannot stop focal seizures in a person with right hippocampal atrophy (small hippocampus or mesial temporal lobe sclerosis) then one should consider four main alternatives (not necessarily this order): -re-evaluation of the seizures with video eeg and neuropsychological testing and an MRI to see if surgery to remove leftover epileptic tissue in the brain is a possibility. -special diets (modified atkins diet, ketogenic diet). It is not a simple task and the risk of long-term use of ketogenic diets in adults have not been studied and documented. -use of the vagus nerve stimulator -use of an brain (cortical) stimulator (aka rns or similar type) such over the right hippocampus - right now you would have some difficulty finding a center to do that but hopefully trials will restart in the near future.
