It : It sounds like you have more than one problem between the nails growing the way they do, the "sausage" toes, and ingrown toenails. It's impossible for me to give you a diagnosis over the internet, but i would recommend seeing a podiatrist. There might be a connection between how your toenails are growing and the fact that your toes are swollen. If there is a circulatory issue, that could affect the normal growing of your toenails. Best thing is to get checked out.

Onychomycosis : Regarding the nails it may be onychomycosis or nail fungus. The small toes may be a congenital abnormality which can sometimes be fixed with surgery.
