All : All of the problems you mentioned are related to your clefts. If you palate has not been repaired completely, you will have communication between your mouth and your nose. The escape of air will explain why speaking will get you out of breath. It also makes your speech sound nasal and hard to understand. There are tubes that vent the middle ear to the back of the throat. The cleft alters the muscles that prevent back flow into those tubes leading to frequent ear infections. The opening in your palate will allow whatever is in your mouth to leak out of your nose. The cleft causes a defect of the bone at the very back of your palate will fail to support the soft tissue allowing it to hang down and cause you to choke. The good news is that all of these problems can be improved with treatment.

More information: I need more information because I do not know if you have been surgically treated and what kind of prosthesis you are wearing to close the exposure. This is most likely a hearing/ent problem, not dental.

Probably: The problem with breathlessness with speaking, ear pain, etc are all probabaly due to the cleft palate history. You should go get evaluated by the speech therapist and ENT doctor on the local cleft team.
