Too many calories: Obesity is the result of intake (food) exceeding output(exercise). There are a few medical conditions that can cause obesity, such as untreated hypothyroidism, and prader willy syndrome. Some medications can also cause weight gain.

Many: Simple answer, too many calories. Long answer, complex disease process, with many alterations in hormones and neurotrasmitters, genetic component, behavioral component, vicious cycle with other comorbid conditions, ex, it can make sleep apnea worse, having sleep apnea often causes weight gain. Usual answer from health professionals, eat right and exercise is often not satisfying with good reason.

Eat more, move less: Anything that causes an obese person to eat more calories or to exercise less will worsen his obesity. Examples: eating as a self-treatment for depression symptoms, eating due to hunger shortly after meals (from high Insulin levels combined with dropping blood sugar levels), not exercising due to being very out-of-shape, not exercising due to being embarrassed by one's public appearance, etc...

Indirectly it can: Excess caloric intake, especially rapidly absorbed sugars can stimulate Insulin activity that drives sugars to be absorbed into cells and storage. That can lead to low sugar in the blood and an irritated nervous system which needs a constant supply of sugar. A few hours after too much sugar you need more or you feel irritable and nervous. Healthier foods are absorbed less rapidly.

Eating/exercise: Obesity is caused by eating too many calories and/or burning too few calories.
Obesity goes away by taking in fewer daily calories than you burn. So eat less calories and exercise more. Weight daily and if your weight is staying the same or going down you're on the right track. If it goes up, then you're not doing enough. Period.
