Ussually : Ussually the inr is considered to be adecuate between 2, 5 and 3 , so I am not clear why is 4 or 5 , or may be I am missing something . There are many reasons for changes in the dosage of nmedications , that go from the medication it self ( produccion , quality, efficacy , byoavalilability etc etc etc ) to issues with the patient , again metabolism, interaction with food, medications and behavior of the medication within the body . The important issue is to keep the inr in the propper range.
