If your doctor works for a federal facility (military treatment center or veteran administration medical center) normally there is not a requirement to be licensed in the same state. I think the most frigthening situation is when someone says that they are a qualified physician but they have not completed medical school (or similar training) and they have never been licensed to practice medicine. In that situation the person would not be considered to be a physician, they would be considered to be a fraud.
