This : This will be a difficult question to answer without additional information. It's possible that this could represent infection, but it could also be something called peritoneal spread of tumor. What was the nature of your friend's operation? Was it elective or emergent? Does he or she have a history of cancer? Was there a suspicion of infection before surgery? Does your patient have a history of medical problems?

That : That is a difficult question to answer without further information. It depends on your friend's medical history. Sometimes cancer can spread as small white nodules seen at surgery. Some types of infections can also cause these white spots. Tuberculosis can rarely cause this, as well as coccidiomycosis rarely can be seen in the abdominal lining (peritoneum). More background history on your friend would be helpful.
