The : The question describes a finding from an MRI examination. Treatment should always focus on patient symptoms. That being said, typical symptoms from a right paracentral and foraminal disc extrusion would include elements of pain, numbness and tingling radiating into the right leg. Conservative therapy would include rest and oral anti-inflammatory medications. Therapy may progress to include an imaged guided steroid injection around the involved nerves. Physical therapy may be involved. Patients who fail to respond adequately to these conservative measures will require evaluation from a neurosurgeon.

Herniated disc: You are describing what is otherwise known as a herniated disc, which can cause severe pain in the back that runs down the leg along the distribution of the involved nerve. 80-90 percent will heal over time. Things like medications, therapy, chiropractic care, and epidural steroid injections can help. A small percentage will require surgery.
