Two possibilities...: Either the diagnosis is wrong or the treatment is wrong. Many over-the-counter anti-fungal creams are not effective for ringworm, despite their advertisements. And if it isn't ringworm in the first place, you'll need an entirely different treatment. So see a dermatologist to get a precise diagnosis and treatment.

Getting from cats?: Cats and sometimes dogs can have the same infection. You could be getting repeated exposure from them. Also, it may not be ringworm, which is just a type of skin fungus. It could be a different rash. If you have no pet exposure, it may be worth seeing your doctor or a dermatologist.
