Chemotherapy: ARDS most commonly occurs after acute injury from sepsis. Sepsis can occur after chemotherapy for malignancies. It has been reported but it is extremely rare for it to occur after chemotherapy without another acute insult such as sepsis.

In some cases: ARDS is a response to a number of lung injuries in which the lung becomes inflamed and fluid accumulates within it. The most common cause related to chemotherapy would be an infection that is indirectly related to the immune system suppression chemo can cause. Less frequently there are some drugs that can directly injure the lung. This is not common but can be seen with bleomycin, vp 16, (etoposide) others.
