Bone vs joint: A hand sprain involves stretching/tearing of the tissues which stabilize the small joints of the hand and fingers. A hand fracture is a break in one of the bones in the hand.

A lot: A sprain is an injury of the tendons or ligaments. A fracture is a broken bone. Both can be a serious problem even requiring surgical treatment. Others may require only splinting or casting and others may not require anything except time and restricted activities in order to heal.
