A few things: It might represent arthritis, injury to the bone such as a stress fracture. Or be in the soft tissues over it. See a doctor and get x-rays to better answer this question.

Foot pain: Foot pain at the medial cuneiform region may be due to a stress fracture or an imbalance of the central band of the longitudinal arch. You should be examined and x-rays should be taken. Are you wearing a surgical shoe or boot to limit the motion in the foot? You may need orthotics to stabilize the foot from excessive pronation.
