At 4 weeks 4 days my hcg was 3, 900. 40 hours later it was 7, 261. my ultrasound isn't until tomorrow, i'll be 5w4d. is that high & could it be twins?

May be: There is considerable variability in HCG levels. Ultrasound exam would be better at detecting twins.See this site and discuss the matter with your doctor. http://www.americanpregnancy.org/duringpregnancy/hcglevels.html.
