What can cause horrible lower back pain that radiates into the buttocks?

Pinched back nerve: A nerve compression in the lower back would be the most common cause; this can be from a slipped disc in the lower back pinching a spinal nerve root, or from arthritis of the bones around those nerves. If this happened suddenly, it means a disc could have slipped at the time the pain started. Treatment is not bedrest (except for maybe a day) but gradually increasing movement along with medications.

Buttock pain: Pain that is felt in this area may originate locally or be referred from other places and conditions. Most hip and buttock pain is mechanical in nature and can be resolved through conservative treatments, but some causes can be serious.
