Painful: The spleen lies just under the diaphragm on the left side so when it ruptures the blood gets under the diaphragm causing significant pain with each breath and sometimes into the left shoulder blade area additionaly blood will run down left side causing pain along left side even into pelvis. It is usually very painful.
