Swelling: Swelling in hands can be caused by arthritis, carpal tunnel, circulation problems, infection, lymphadema, salt retention, wrapping from tight bandages.

Some suggestions: There are multiple causes, including rheumatic disease, infections, poor circulation, arthritis. Sometimes the initial workup does not yield a diagnosis. I would suggest that you review your records with your primary doctor and discuss all these possibilities. An ESR is a general screening test for inflammation.
