Not shingles: The information you provided is not consistent with shingles. You should see your doctor to figure out what may be causing the lesion.

Antiviral meds: Getting the Acyclovir early in the course may help but often does not stop the immune response causing the skin inflammation and pain. Once the inflammatory reaction is in full course the antiviral meds do you not much good. A full medical workup is important to rule out any systemic disease associated with a repeat outbreak of shingles.

Shingles: If rash in both breast, then there is no chance it is shingles. Please see your doctor.

Not typical: Shingles is usually unilateral, and the rash over the breasts does not seem to fit the severe neck pain issue. Cannot advise therapy, but suggest you have a full exam and appropriate testing.
