Visit a vascular: Doctor for the varicose veins. They could evaluate you and give you options. If you are heavy (weight wise) losing weight will help take some pressure off the veins. Compression hosiery if there is any degree of swelling can help. If fat around ankles is an issue it can be surgically removed.

Phlebologists: Some doctors specialize in conditions of the veins. They are called phlebologists. They are typically better equipped to treat swelling, varicosities and other conditions with small in office types of procedures.

Edema of legs: Edema of legs with varicose veins can be due to venous insufficiency. You should make an appointment with your primary who can refer you to both a podiatrist and a vascular specialist. Fat legs or fat ankles should also be addressed by specialists. Usually this is due to some type of venous failure, lymph edema or another type of collagen- vascular disorder.
