Not sure: It is interesting that you receive that information. I am not aware of grade 3 venous insufficiency classification. Ther is a ceap classification. C3 corresponds to patients that have edema as a consequence of venous insufficiency, however gets a bit more complex when add the other components. Would be better for the general publc to refers as mild/moderate/severe.

Knee level reflux: Grade 3 venous insufficiency refers to an older method of severity scale when it was thought venous insufficiency originated from the saphenofemoral junction. It graded the retrograde flow to be at the junction, the thigh, the knee, below the knee or at the ankle. Grade 3 would indicate retrograde flow or venous reflux down to the knee level.

Another answer: There might be another answer to your question, although i completely agree with what dr. Presant said. Another word for venous insufficiency (vi) is reflux but there is also chronic venous insufficiency (cvi) which has to do with how refluxing veins affect the leg. Stage 3 cvi would be a venous leg ulcer. These days, you don't hear people talking about grades of vi or cvi much any more.

Venous dz: Chronic venous disease today is described using the ceap classification. Most will use the c (clinical )as a way describe the patients pathological condition . C3 involves swelling, C4 skin changes ( brawny induration) without ulcer, C4 healed ulcer , and c 5 active ulcer. This classification allows us to have shared terminology in regards to diagnosis and treatment.

Grade 3: There is a classification grade 4 venous disease-CEAP,, And if you are referring to that it means that you have ankle edema(Swelling) secondary to the venous disease.
