How do you know if a brain aneurysm requires immediate surgery if it is not bleeding?

Depends: Urgent treatment for an anuerysm is recommended if the aneurysm changes in size or shape on an imaging study or causes new headache or changes in vision or hearing.

Size: Aneurysms larger than 7mm should be treated sooner rather than later. Unless an aneurysm has bled, there is no reason to treat emergently. The only other consideration is if the aneurysm is growing quickly, it should be treated sooner rather later!
