Muscle?: Might be a strained stomach muscle. Treat the cough and let the stomach rest and it might go away on its own.

Muscle strain: You may have torn or strained yous abdominal muscles. Try some heat and ben gay. If not better see you pcp.

Previous surgery?: If a patient has adhesions in the abdominal cavity from previous surgery, there could be some pain from increasing that pressure when coughing.
