That's unusual: Unless you've abstained from sexual intercourse until recently from the time of your tubaligation, it's unlikely that it would fail 3 years later. However, if you're concerned about a possible pregnancy, just take a store-bought urine pregnancy test. Testing is the only way to rule out pregnancy at this point.

Pretty untlikely: Tubal ligations are almost 100% effective. You could be pregnant if the doc made a mistake in surgery and didn't really tie your tubes, but tied some ligament up, or if the tubes grew back together. The way tubal ligations are done these days, in the U.S., both of those possibilities are incredibly unlikely.
