Unlikely: A high ph is alkaline, not acid. Any diarrhea can cause diaper rash. There are many causes of diarrhea at this age - infection, malabsorption, celiac etc. Please follow up with your doctor; see a ped GI specialist if recommended. Meanwhile clean with soft cloth and soap/water, and use appropriate antibiotic or antifungal ointment for the rash. Lactose intolerance is uncommon under age 6.
