PSA?: Is test used for people with prostatic hypertrophy , if it is elevated they do biopsy . The result is not always accurate.

Not always: Around 20 percent of prostate cancers are detected in men with normal psa. Most urologists use age adjusted cutoffs which are lower in men below 60 than the lab cutoff of 4 ng/ml. In rare cases men have very aggressive (high grade) cancers that produce little psa yet may be advanced at the time of diagnosis. So while most men with prostate cancer will have some psa elevation, some do not.
