Go to ER: This could be several things. The most concerning diagnosis with an abnormal lie of the testicle and sudden onset pain is testicular torsion, when the testicle turns inside its layers of connective tissue and strangulates its blood supply. Other possibilities are equally dangerous and should be investigated as soon as possible.
