What are the really thick bulges at the joints of old people's fingers?

Osteophytes: You are probably referring to heberden's and bouchard's nodes. These typically present as two bumps on side of the finger joints. They are extra bone forming in response to wear and tear stress.

Bone spurs: As the cartilage in the joint wears down and becomes thinner, the adjacent bone grows. The bone spurs are not always painful but can limit motion. The condition is called osteoarthritis.
