Terminal: Most patients with dementia, especially alzheimer's disease, eventually die from an infection, especially pneumonia. Many families are realistic about prognosis, and provide a dnr instruction to allow a death with dignity.

It depends: Pneumonia can cause discomfort but that can be treated with analgesics (morphine), antipyretics (tylenol) and oxygen if necessary. Antibiotics may not be necessary to provide comfort and may not be effective in treating pneumonia.
