LUTS: This man should drink more fluids daily. He likely has incomplete bladder emptying which causes the urine to stagnate in the bladder and become more smelly. He should be on treatment for this problem.

Here are some ...: Solutions include changing diaper soon after peeing & pooping, removing the soiled clothes soon after change, properly ventilating the room, daily bathing the patient, treating UTI, changing catheter timely, & judiciously spreading deodorants. In fact, these should be a part of standard care in such facility.
