As early as month 5: The many hormonal changes in pregnancy include higher estrogen, that leads to production of high prolactin, which in turn causes breast gland development and enlargement. Some women notice small amounts of breast milk leaking as early as 14-16 weeks. Others do not have any until after delivery. Stimulation of breasts, children crying, prior breast feeding may predispose to earlier breast milk.

Usually not. : Some women produce milk "ante-partum, " before delivery. There's no particular timing for that. Most don't begin producing milk until "post-partum, " or after delivery. At that time women start producing colostrum, a liquid high in protein which supplies the newborn with antibodies. Around day 3-4 after delivery, in response to nipple stimulation and the hormone progesterone, the "milk comes in.".
