To relieve symptoms: Stage IV means that the cancer has spread to distant organ beyond the lung. Unfortunately, it is incurable disease, although it is treatable.The role of radiation in a stage IV disease is limited to relieve symptoms eg- to treat painful metastatic disease to the bone, or to shrink the tumor when the cancer encasing vital organ- like major blood vessels/main airway. Chemo is the mainstay therapy.

Depends: In stage 4 (metastatic), radiation is usually given to debulk the tumor together with radiation, or for local benefit such as pain, relief of obstruction etc.
