Collagen vascular: While most cases of chronic urticaria are idiopathic and are not related to major illnesses, there are cases that are caused by collagen vascular diseases such as lupus. Hepatitis can also cause hives. I have seen it with mycoplasma infections and very rarely it can be associated with certain malignancies.
