None: Amphetamines are banned substances by the ioc, ncaa, mlb, nba, nfl, and nhl. They increase your risk of cardiovascular events and should only be used when medically indicated for very specific conditions. Even then you may have to gave special clearance from these governing boards to use them.

None currently: I would note that some athletes are prescribed forms of amphetamine for other conditions like adhd. I once coached a football player whose adhd was so bad that he would forget the play between the huddle and the line of scrimmage when he was not on his medicine. Amphetamines were once used by boxers and wrestlers trying to cut weight. This practice has not been endorsed for several decades.

They are illegal in any competitive sports environment where drug testing would occur. They have no legitimate or safe indications for use in sports. No sports medicine provider values their integrity, credibility for medical license would prescribe or recommend them

Mostly banned : Amphetamines are generally banned for organized sports participation. Certain medications such as ADD medications like Ritalin may qualify with a therapeutic use exemption

None: Sports medicine will not use amphetamines. it is a banned substance in competitions. Unless you are diagnosed and treated by a non-sports physician for a condition like narcolepsy or adhd and there is significant history and documentation to back that up.
