Not known: Decades ago iodine deficiency was a common cause. These days the cause of benign enlargement of the thyroid (goiter) is not known. A substernal goiter is an enlarged thyroid that extends from the neck into the chest. Anyone with an enlarged thyroid should have an ultrasound and most likely a needle biopsy of the gland.
