Eye Alignment: Double vision - when present with both eyes open is caused by eye misalignment. Astigmatism is a corneal or lens irregularity in focus and can present with a ghost image (double) with either eye open. Also, cataract can cause astigmatism and double vision with either eye open. It is very imporant to distinguish between single and both eyed double vision.

Appearance: Double is usually two images separate either horizontally, vertically, or obliquely. Not necessarily blurred unless also uncorrected refractive error. Astigmatism is a refractive error causing slight distortion of an image in one direction. Depends on amount.

Astigmatism: Double vision can be due to optical or ocular alignment abnormalities. When light rays do not focus on the retina properly double vision can result . If the eyes are not moving in sync together double vision can occur as well.
