Constipation: Very common. Take increased fiber and increased liquids. Stool softeners ( MiraLAX (polyethylene glycol) or kristalose) helps. Consult your OB gyn.

Scratch/fissure: Bright red blood on your toilet tissue or seen on the outside of a stool is from a crack/fissure in the outlet. Hard/bulky or frequent stools can cause the crack & it will take a while for it to heal. Softer and or scheduled stools can reduce the load on the outlet. Miralax or similar products are stool wetting agents and can help over the long term. They can be taken daily during pregnancy.
