Sharp pains: I don't think i would jump to a pancoast tumor right away with this history, especially if the ct was negative. There are other things that can cause pain that are not picked up on ct, like cartilage and muscle irritation. Consider trigger points. Also have thorough exam of shoulder b/c pain could be referred from there.
